These gamesheets must be fully completed, with the referee's OSA number, signature and printed name. We have approved hundreds of Exhibition game permits over the last month with poor returns of gamesheets.
We all must maintain these gamesheets in the event any incident is recalled, up to 15 years from the date the game was played. It is imperative that we maintain all our records.
If an exhibition game has been cancelled before the permit has been approved, please notify the district office so that we do not charge you for the event. Not to mention it is more efficient for all of us!

Some important notes re-iterated at the last meeting:
In District: Games between 2 YRSA CLUBS [example: VSC vs. WOODBRIDGE]
One Team will be designated as the "HOME TEAM" and must apply for an AHEG to host the Exhibition Game
This applies FOR teams within club playing each other: One must apply to HOST exhibition Game only....
a) Game taking place within the boundaries of the YRSA
The "AWAY Team" MUST apply for a TRP to the Exhibition game, Club to submit their approval normal.
b) Game taking place outside the Boundaries of the YRSA
The "Away Team" will require an ATF and should have their Club submit their approval as normal.
Out of the District – Between an YRSA Club and Out of District Club
YRSA Club as Host Team: the YRSA will approve the AHEG form within 2 weeks of the receipt of the AHEG request from the Club. A copy of the "Approval Receipt" will be mailed back to the Club Administrator and forwarded to the traveling team's District on the day the AHEG is approved. The traveling team's District will then approve the ATF.
YRSA Club travels outside the District – The Hosting Team's District will approve the AHEG first and then will forward the approval onto the YRSA for acknowledgement. Then the YRSA will approve the ATF form with 2 weeks of the receipt of the approved AHEG and ATF request from the Club. A copy of the "Approval Receipt" will be emailed back to the Club Administrator on the day the AHEG is approved.
Game sheet
The YRSA & Club – Teams that hosted the Exhibition Game must mail or scan/email the complete game sheet back to the District with 7 days of the game. The game sheets must include the following PRINTED CLEARLY:
All players' names & OSA #'s
All Teams Officials' names & OSA #'s
All Game Officials' names & OSA #'s
All Special Incident Reports, Caution, Dismissal & Referee Assault Forms
All Personal injuries must be reported in written form to the District & Club ADMIN
Sandra Colica - must retain the original or a copy of both Games Sheets and staple them to the Approved Application forms for a period of 7 -10 years....
